Monthly Primary Care Memberships

Flexible options for individuals and families, with a set number of primary care visits included each month.

Individual Membership → $69/ Month

For one enrolled patient.

2 standard primary care visits per monthly billing cycle
Telehealth appointments for concerns appropriate for virtual care
Additional standard visits: $49 each

Family Membership → $159/ Month

For 2–5 enrolled household members.

4 standard primary care visits shared among enrolled members per monthly billing cycle
Telehealth appointments for concerns appropriate for virtual care
Additional standard visits: $49 each

Large Family Membership → $229/ Month

For 6–10 enrolled household members.

6 standard primary care visits shared among enrolled members per monthly billing cycle
Telehealth appointments for concerns appropriate for virtual care
Additional standard visits: $49 each

Membership details

Visits are subject to appointment availability. Unused visits do not roll over. Home visits, laboratory testing, imaging, procedures, medications, and supplies are billed separately. Memberships are not health insurance.

Currently available care: Telehealth throughout Utah and Wyoming, with home visits available separately in the Eagle Mountain area.

Chronic Disease Management → $95

Sports / School Physical → $45

Established Patient Visit → $85

Medication Management → $65

Preoperative Evaluation → $125

Sick Visit → $85

Annual Physical Exam → $125

Complex / Extended Visit → $150

A preventive physical focuses on preventive care. Evaluation or treatment of separate medical concerns may require a separate service charge.

Telehealth Visits

Convenient virtual visits for concerns that can be appropriately addressed online.

Simple Telehealth Visit → $60

Extended Telehealth → $85

Home Visits

Personalized care at your location within our designated service area. Availability and travel fees may vary by address.

Initial Home Visit → $175

Extended / Complex Home Visit → $200

Established Patient Home Visit → $135

Addit. Family Member, Same Location → $65

Same-Day Acute Home Visit → $150

The additional-family-member rate applies to another patient seen during an already-scheduled home visit at the same address. Please contact us to confirm your address is within our service area and to learn about any travel charge.

Important Pricing Information

Prices listed are self-pay visit fees. Testing, procedures, medications, and other services may cost extra. Additional costs will be discussed before services are provided. Insurance patient responsibility depends on plan benefits and applicable coverage. Same-day appointments are subject to availability.

For uninsured or self-pay care, you may have the right to a Good Faith Estimate of expected charges under federal law. Please contact our office if you would like an estimate before scheduling.
